Outdoor Ed forms are due by this Friday 9/21!!! Please get them in as soon as possible.

There are three forms plus a check:

1. Permission slip

2. Health Form

3. Medication Form ***Even if your child doesn't take medications, there is a section at the bottom left to indicate any medication allergies, as well as medications our School Nurse may give for comfort. This form must also be signed.
