Team Assignment Due April 18:

Use this template to share a PDSA of a visit you tested using teledentistry. You will present your slides in small breakout rooms during the April 23rd Learning Session.

Teams are required to submit monthly data on up to 3 measures:

1. Count of synchronous teledentistry visits in the reporting period

2. Count of asynchronous teledentistry visits in the reporting period

3. Count of all teledentistry visits (synchronous and asynchronous) billed for during the reporting period

Due Dates:

• February 7 - Baseline data for July 1 - December 31, 2024

• March 7 - Data from February 2-March 1, 2025

• April 11 - Data from March 2-29, 2025

• May 9 - Data from March 20-April 26, 2025

• June 13 - Data from April 27-May 31, 2025

• July 11 - Data from June 1-28, 2025
