Team Travel Forms

Traveling with a school team is always exciting. But, we have to make sure it is always safe as well. To ensure the health and safety of our student athletes during the trip we require the advance completion of the following travel forms.

In regards to finances, this year money is not to be collected by coaches or parents. For tracking purposes, it needs to be paid directly to the main office at Tricentia Academy. The money collection form below should be completed by the coach in advance and will be used at the office to track payments.
