START EARLY GETTING YOUR 6TH GRADER READY FOR 7TH GRADE!
ADDITIONAL VACCINATIONS ARE REQUIRED!
This is a notification of Texas immunization requirements for students that will be entering 7th grade. As your student receives these immunizations, please provide the school a copy of the record to update your child’s school records.

The Texas Department of State Health Services now requires all students entering the 7th grade to have the following immunizations after turning 11 years of age:
Tdap - 1 dose is required if at least 5 years have passed since the last dose of tetanus-containing vaccine
Meningococcal - 1 dose is required on or after the student’s 11th birthday
Record of receiving these vaccinations must be received by your child’s campus prior to the start of the 2022-2023 school year. Please review your child’s immunization records and make plans to have them up to date. As the start of a new school year approaches clinics begin to have appointments and vaccine clinics that fill up fast, so take the time to plan ahead.
To obtain these vaccinations, please contact your child’s health care provider or the Tarrant County Health Department at

Remember to provide a copy of your child’s updated record to their school as he/she receives these immunizations. If your student does not have these vaccines before the first day of school for 2022-2023 school year, they will be sent home.

School Immunization Clinics
Passport Health/Staying Healthy Medical Services will be conducting a children’s vaccination clinic to get your students up to date for the school year.
July 16, 2022- 9:00 am to 11:00 am- NISD Administration, 2001 Texan Dr, Justin
July 22, 2022- 3 pm to 5 pm - Eaton High School PAC, 1350 Eagle Blvd, Haslet
July 27, 2022- 8:00 am to 10:00 am- Medlin Middle School, 601 Parkview Dr, TC
August 3, 2022- 3: 30 pm to 6 pm- PIKE Middle School, 2200 Texan Dr, Justin
Only students attending the school district that is hosting the clinic can be seen.
A current vaccine record or note from school stating missing vaccines is required.
Parent or guardian must be present. Children 4-18 years old can be seen. This should not replace any well child checkups at the pediatrician. (Exceptions made for younger children entering school district head start program.) Older children who resist shots and children with special needs may not be able to receive services depending on their cooperation. Please refer to your pediatrician if needed.
Staff and supplies are limited and there is no guarantee you will be seen. The clinic is on a first come basis and may be discontinued when supplies or time runs out.
We accept the following forms of payment:
Aetna, Blue Cross Blue Shield, Cigna, Humana & United Health Care PPO plans will be accepted.
**WE CANNOT ACCEPT AETNA & BLUE CROSS GOLD, SILVER, OR BRONZE PLANS OR HMO PLANS WITH ANY INSURANCE COMPANY.**
A paper photo copy of insurance card and driver’s license MUST be provided to receive vaccines at no charge.
Medicaid/CHIP
No charge for vaccines. Must provide a paper photo copy of card.
UNINSURED
$10.00 PER VACCINE
($40 maximum per child.)
Vaccines available: Chicken Pox, DTaP, Hepatitis A, Hepatitis B, HIB, HPV, Meningococcal, MMR, Polio, Pneumonia, TDaP
