(Hereinafter, the “Organizers”)

ASSUMPTION OF RESPONSIBILITY, RISKS AND LIABILITY WAIVER

(Hereinafter, the “Release”)

BY SIGNING THIS LEGAL DOCUMENT, YOU WILL BE GIVING UP CERTAIN LEGAL RIGHTS, INCLUDING THE RIGHT TO SUE – PLEASE READ CAREFULLY.

Event: Lakers Soccer 7’s Tournament (hereinafter, the “Event”)

Date: Saturday, May 25 – Sunday, May 26, 2019

Location: Nipissing University Turf Field– 100 College Dr.

By signing this agreement, I UNDERSTAND THAT AS A PARTICIPANT, ALCOHOL AND/OR ILLEGAL DRUGS MAY NOT BE CONSUMED IMMEDIATELY PRIOR TO THIS EVENT OR AT ANY TIME DURING THE EVENT.

ASSUMPTION OF RISKS and ASSUMPTION OF RESPONSIBILITY

I REALIZE THAT THERE ARE RISKS INHERENT IN MY PARTICIPATION IN THIS EVENT.

I freely and voluntarily accept and assume all such risks, dangers and hazards including, but not limited to, the possibility of personal injury, death, violence and/or property damage or loss resulting from my attendance at, and/or participation in, this Event and/or related activities.

I accept my responsibility to abide by the laws of Ontario, to ensure that I have adequate medical coverage, to protect personal possessions, and to obey all the rules set out for this Event by the Organizers.

LIABILITY WAIVER and INDEMNIFICATION

I hereby release and discharge personally and on behalf of anyone claiming through me, the Organizers, including all related and affiliated persons, and their directors, officers, staff, students, volunteers, agents, trainees, and/or employees (the “Released Parties”) from all claims at any time that I or anyone claiming through me may have arising out of participation in the Event and/or Event related activities. Moreover, I will not bring a claim related to these activities against any person or entity where such a claim may lead to a claim being brought against any of the Released Parties for contribution or indemnity.

I also hold harmless the Released Parties from claims of any kind that I or anyone claiming through me may have arising out of my participation in the Event and/or Event related activities.

This release shall be binding upon my heirs, beneficiaries, and/or personal representatives and shall inure to the benefit of the Released Parties, their successors and assigns. If any provision herein is held to be invalid or unenforceable, the remainder of this Release shall be construed as if that provision was not contained herein.

By signing this agreement, you are confirming that you have read and understood the following:

1. You can read and understand English.
2. You understand the purpose of this release.
3. You understand that this Event has inherent risks, and that you are willing to assume these risks.

IF YOU DO NOT AGREE WITH ANY OF THE ABOVE, PLEASE DISCUSS THIS RELEASE WITH THE PRIMARY EVENT ORGANIZER ADMINISTERING THE RELEASE.
