A.   MA Students

• For your Program of Study (POS), please let  Dr. Jose Perico Esguerra sign in the middle box. Also, let Dr. Esguerra sign as Conforme your  Form 5A. Submit a hard copy of your POS and Form 5A to the NIP Administration Office through Ms. Rose Anne Pilar.
  
  

B.   MS and PhD Students

  B.1   Graduate Students without a Research Adviser

• For newly admitted students, please email  Dr. Kristian Hauser Villegas / Dr. Miguel Sulangi for your Program of Study. Let Dr. Villegas/Dr. Sulangi sign the middle box.

  B.2   Graduate Students with a Research Adviser

• Please note that in the Program of Study (POS) the students must identify their specialized, breadth and general electives. Have your updated POS approved by your Research Adviser prior to sending it to the NIP Director.

Instructions to Graduate Students:

1. To update your Program of Study (POS), please have it signed by your research adviser (middle signatory in the POS).
2. Let you research adviser sign in the left corner of your UP Form 5A as conforme to the courses you intend to take this semester. Indicate in your Form 5A, the following: "Research Adviser Conforme".
3. For POST ADVISING, please submit a hard copy of your approved POS and UP Form 5A to the NIP Office (c/o Ms. Rose Anne Pilar).

Notes:

1. All Form 5's and 5A's are signed by the NIP Director and Graduate Committee Chair, Dr. Wilson O. Garcia.
2. Check that the POS entries are complete.
3. As a general rule, the time limit for the completion of all degree requirements shall be no more than x years. Residence shall start from the time the student enrolls in a graduate course in the Program and shall include all leaves of absence from the Program.

• MS Physics: x = 5
• PhD Physics (entered the program with an MS):  x = 6
• PhD Physics (entered the program with a BS): x = 8
