Co-op Application Form

Students who have selected co-op on their option sheet or have Co-op in their timetable must complete the Co-op Application Form.  Please be as specific as possible in terms of what you would like to do for your placement.  

Please note that first choice of placements are NOT always possible so we do ask that you rank your top three types of placements.  

Lastly, please ensure you have a conversation about transportation with your parent/guardian as to how you will get to and from your placement each day as transportation is NOT provided.  Getting to and from your co-op placement is your responsibility and we cannot guarantee a placement within walking distance from the school.
