Medication Forms
If you need to keep medication on site for your child, please read the below information.
Medication paperwork must be received in the office with all the required signatures before any medication may be kept on site for your child. Paperwork can be faxed to the office (919-882-8872), emailed to Heather Townsend (109.htownsend@nhaschools.com), mailed to the school, or dropped off in person during office hours. Now is a great time to get started on getting together all the necessary documents, signatures, and prescriptions for keeping medication on file at the school for your child. Below are descriptions of the required paperwork.
ALL MEDICATION
A Medication Administration Permission Form is required for EVERY medication that is on site at RCA.
If your child needs more than one medication on site, a separate form must be filled out for each medication.
ALL medication requires a parent AND doctor signature on the Medication Administration Permission Form, per NC Law (including OTC medication and remedies).
We require all medication to be on file in the front office.
All medication must be in brought to the school by a parent and it should be in its original packaging; the name on prescription medication must match the child for which it will be on file.
Expired medication cannot be accepted.
If the medication is to be administered on a regular basis, please communicate with your child’s teacher and the front office staff so a plan can be made that accommodates your child’s medical needs as well as the school schedule.
ASTHMA MEDICATION
An Asthma Action Plan is required for all asthma medication.
Asthma Action Plans must include a doctor signature.
If your provider has their own version of an Asthma Action Plan (different than the file attached), we may be able to accept it.
If your child uses an inhaler that requires a spacer, one must be provided and kept on file with your child’s medication.
ALLERGY MEDICATION
A FARE Form is required for all Allergy and Anaphylaxis medication.
The FARE Form must include a parent AND a doctor signature.
If your child requires an epinephrine injector to be kept on site, the family must provide TWO injectors in their original packaging with the child’s name on the prescription tag.
If your child’s FARE Form states that Benadryl shall be administered before/with Epinephrine, please plan to provide the Benadryl to be kept on file for your child (an additional Medication Administration Permission Form will need to be completed).
