Paper copies of all forms can be found in the front office. Electronic copies are available at the link below. All form MUST be completed on a yearly basis prior to participating in any practices or competitions.
Physicals must be up to date meaning if the date of the physical is more than 365 days than the current day it is out of date.

Student athletes have to follow the policies listed in the sports form packet. Electronic copies of the policies are included in the link below they cover the academic eligibility, behavioral eligibility, and jersey contract.
