Waiver and Play Based Assessment 

The waiver will be on this page no later than January 10, 2024. 

Please download the document and complete the document.  The form should be returned to the school (Hill and Plain or Northville Elementary School) your child would attend either in person or mailed.  Once the form has been received, you will receive a confirmation email letting you know it has been received.    The waiver should be completed, and received by the building administrator no later than 4:00 p.m. on February 23, 2024. NOTE:  

The waiver process is now closed.  Students who have not applied for a waiver and will have turned five by September 1st will be eligible for kindergarten for the 2025-2026 school year.  

Submission may be done in-person,  mail or as an email attachment.

Families will receive communication from the building administrator the week of March 4 regarding the next step - the student assessment portion of the waiver.

About Our Play Based Assessment

Once the waiver is submitted, the child will be invited to participate in a small group play based assessment.  This 75 minute session will allow us to get to know each child better in a school-based environment.  Additionally, parents will be discussing their child with a certified staff member to help us create a clear picture  of the child's readiness.

Sessions will be held during the week of March  11th and 20th at John Pettibone Community Center.   Parents will be contacted to set-up a date and time. 

Families will receive final waiver notification on Monday, March 25th.
