What are the office hours of the Registrar's Office?
Monday to Friday, 8:00 AM – 5:00 PM, excluding holidays. No Noon Break.
Saturday, 8:00 AM - 5:00 PM. Lunch break is usually from 12:00 PM to 1:00 PM
How can I request my Transcript of Records (TOR)?
Before you can request, your admission documents must be complete.
Secure a request form from the Registrar’s Office or download it online (if available).
Fill it out and submit it together with your official receipt of payment, ID Picture (1 pc. passport size with name tag and white background), and the required no. of pcs. of Documentary Stamps.
Processing time typically takes 3 working days.
How do I request for a Certificate of Enrollment, Certificate of Graduation or any Certificates which the office issues?

How to request Student Permanent Record, Certificate of Registration (COR), and Student Grade per Semester for Scholarship purposes?
Proceed to the Registrar Office and ask for the price of those credentials.
Go to cashier office to pay the corresponding price and present the Official receipt to the Registrar Office.
Processing time typically takes 5 to 10 minutes.
How do I apply for a Transfer of School?
Before you can apply, your admission documents must be complete.

Can I authorize someone to file/request documents on my behalf?
Yes. An authorized representative must present an authorization letter (notarized if Honorable Dismissal), a photocopy of your valid ID, and the document owner valid ID.
Can I authorize someone to claim documents on my behalf?
Yes. An authorized representative must present a notarized authorization letter, a photocopy of your valid ID, and the document owner valid ID and the Original Claim Stub.
How do I apply for graduation?
Before you apply, make it sure that you are a graduating student.
Proceed to the Department and ask for a hardcopy of Application for Graduation. Fill out the graduation application form.
Submit it to the Registrar’s Office with complete requirements before the posted deadline . Accomplished clearance must be attached, photocopy of Approval Sheet, and photocopy of Abstract (if Graduate School student) .
Do I need to bring my clearance slip when enrolling?
Yes. Bringing your clearance slip helps fast-track the approval of your Certificate of Registration (COR).
