DOT PHYSICALS
The office is now located next to Flaget Hospital. Per 702 KAR 5:030 we must designate a physician to perform your physical. In addition we may not accept physicals from a Chiropractor. This means we can no longer accept a physical from Dr. Bowles or any other organization outside of CHI Ambulatory Care per regulation.
NO PHONE POLICY
Phones are not permitted to be used or looked at while the bus is on the road. All phones need to be stored in your pocket, purse, storage compartment or somewhere else completely out of view when you are driving the bus. Smart watches shall not be looked at while the bus is in motion.

Prohibited Items
ALL illegal items on your bus needs to be removed. If you have an item on your bus that was not on the bus when it was delivered new - then it needs to be removed.
Anything hanging from your seat, trash bags, trash cans, aerosol cans, window clings, brooms, etc are illegal. Please bring your bus into compliance.
