Form 1095-C, Employer-Provided Health Insurance Offer and Coverage, is available to eligible employees upon request. Current employees may access the form through their Etrieve account. Former employees have been mailed a copy.
To request an additional copy, please contact Melissa Childers. Requested forms will be provided within 30 days.
Form 1095-C is not required to file your federal income tax return, but should be kept with your tax records.
This notice is provided in accordance with guidance issued by the Internal Revenue Service.
