Form D
The seminary is the administrator of the internship as a credit course of your degree. So all internship forms are returned to the contextual ed office of your seminary. They will assemble everything into a final report that they then pass along to the candidacy committee. If you have questions on what to write in the internship evaluations/Form D, please contact your academic advisor and/or the contextual ed office of the seminary. They should be able to walk you through that.
