How to register on the Quadrant App
Prior to arrival for your appointment, you must register on the Quadrant app. This is the laboratory that will read the tests and report the results. Once you are at the home page for Quadrant, please select Register. You will set up a login and password to create your account. Follow the prompts until you complete the registration. You may decline the social security information and omit the driver's license information. Also, because there is NO CHARGE for testing, you do not have to enter any insurance information(select no coverage). You must agree to all three terms of service on the final page to complete the registration.
IMPORTANT! When registering your child, please select their school from the drop down menu. Do not select North Colonie School District. This way, the student's result is shared directly with the building nurse. For staff, be certain to choose North Colonie-staff.
