General Elementary Information

• All Parents must complete online the Annual Student Health Update Form before school starts each year.

• Medication Consent Form - any child taking medication at school

• Food Allergy Diet Modification Form - any child with a food allergy

• Allergy Action Plan- any child with a severe allergy

• Asthma Action Plan - any child with asthma

• Seizure Action Plan - any child with a seizure disorder

Jump Start / Preschool

• Up to date immunization card is required prior to the first day of school.

• School Physical is required prior to the first day of school completed by your provider.

• Annual Student Health Update Form

Transitional Kindergarten/ Kindergarten

Requirements for Kindergarten are linked below, you will also receive paper forms at Kindergarten round up.

• Up to date immunization card is required prior to the first day of school. (DTaP, Polio, MMR, and Varicella required after 4 years of age.)

• School Physical is required prior to the first day of school completed by your provider.

• Annual Student Health Update Form

• Vision Screening Form

• Dental Screening Form

• Lead Level- this will be indicated on your physical form
