β Health Changes: Please let me know if your child experienced any key changes to their
health over the summer.
β Physical Exam Forms: If your child had a physical exam this summer, please forward a
copy to me.
π School Medication Requirements
An updated doctorβs order and parental permission form are required every school year for any
medication administered at school.
This includes (but is not limited to):
β Daily medications (or office equivalent)
β Inhalers & Asthma Action Plan (or office equivalent)
β EpiPens & Allergy FARE forms (or office equivalent)
β Seizure medications
β Migraine medications
β Diabetic medication & monitoring supplies
Important: Per North Attleborough policy, all medication must be dropped off by
an adult.

You can bring medications and completed forms on the first day of school, or take advantage
of our early drop-off hour:

Student Orientation)
