This will help organize your medical records and allow you to get the most out of your pharmacy visit! Most of your health information can be found on a recent hospital discharge paperwork.

Meet your pharmacist!
Most meetings takes 15-30 minutes. This gives you time to explain your current medical conditions in more detail, ask questions you have concerns about, and learn about your medications in depth. The pharmacist will listen and answer questions. We will talk about how to take your medications, potential cost savings opportunities, and tell you recommendations to your list to optimize your care.

Follow up
After the meeting, you will receive an email with a recording of the meeting so you can refer back to it as well as list of potential recommendations to show your Doctor at the next visit. You will also receive an easy to you medication list with a pill box picture. We encourage you to use our recommendations as a conversation starter with your doctor.
If urgent care is needed, a list of recommendations will be faxed to your Doctors office so that they can review and make changes. Feel free to reach back out as your medical condition changes or as new meds are added! We recommend following up at least once per year or earlier if your health condition changes.
