• To enroll in Trio HMO plan, you must live or work in the Trio ZIP code service area

Access+ HMO plan

• The lowest-cost broad network HMO plan available to CalPERS members

• Offers the option to self-refer to specialists within your primary care physician’s medical group

• Gives you access to the largest provider networks in California—more than 10,000 primary care physicians, 270 hospitals, and 34,000 specialists
