If you athlete has more than one game (on different days) you must complete this form twice with the correct event link. Links to individual events are listed below.
If you have a child that is under the age of 18 and does not have a driver's license the information still needs to be completed for them to get onto the Air Force base (just use NA on the driver's license number and state box.
The deadline is set and no exceptions will be made. Reasons for the deadline is the Air Force base requires 10 working days before the event to get background checks completed for people entering onto the base. This is a United States Military rule not a Grand Forks Public Schools rule.
If access is denied to get onto the Air Force base (you will be informed by the school) the school district can not change or help to have the decision reversed.
