Welcome to the Continuing Healthcare package.

Our aims within this package are to:

• Explain the system changes e.g. Integrated Care System

• Explore basic explanation of the legal background to NHS Continuing Healthcare, defines key terms, introduce to the national tools and outline the overall process of considering eligibility of NHS continuing Healthcare

• Explain key links to wider requirements including obtaining consent, applying Mental Capacity Act and, most importantly ensuring person centred approach

Our objectives are to:

• Help you understand how a persons care package is commissioned

• Explain how the process is a patient centred approach which involves family and carers

• Show you the professionals involved in this multi - disciplinary decision

• Reinforce the importance of communication, professionalism, managing expectations, record keeping ( records that are assessed are the patent record 3 months before decision)
