Episodes of care are holistic assessments which enable you to demonstrate your progression across a number of platforms. Effective communication and relationship management skills underpin all aspects of care. This assessment must be completed by the practice assessor.
Along with your practice supervisor / practice assessor you will identify an appropriate episode of direct care involving meeting the needs of a person/family receiving care. Professionalism underpins all aspects of your performance.
You will carry out 2 separate episodes of care. You need to pass both assessments to progress.
Following the episode of care you will reflect upon the experience. The practice assessor will document if you met the relevant proficiencies.
By the end of this placement you are required to have completed and passed 2 episodes of care assessments to progress.
What are the Professional Values Assessments?
You are required to demonstrate high standards of professional conduct at all times during placement.
You should work within ethical and legal frameworks, and be able to articulate the underpinning values of The Code (NMC, 2015).
The Professional Values reflect a number of proficiency statements and provides evidence that these proficencies are being met
At the end point assessment you choose one example from practice to demonstrate how you practise within The Code (a different example for each assessment). Confidentiality in relation to people receiving care and service providers should be maintained.
These should be referenced and it is recommended no more than 500 words.
By the end of this placement you are required to have completed a mid-point Professional Values Assessment and a final-point Professional Values Assessment to progress
What is the Medicines Management Assessment?
You are required to undertake a medicines management assessment, This assessment must be completed by the practice assessor.
Yourself and the practice assessor should identify the appropriate time to complete this assessment. There is only one opportunity for this assessment. The student should be allowed a number of practice opportunities to administer medicines under supervision prior to this assessment.
The aim of this assessment (level 5) is to ensure you can safely administer medicines to a group of patients/service users or a caseload of patients/service users in community settings.
By the end of this placement you are required to have completed and passed a Medicines Management Assessment, you will only have one attempt at this.
