NEW TO ST. THOMAS AQUINAS?
Any student who previously attended another highschool prior to attending St. Thomas Aquinas is required to fill out the OFSAA TRANSFER APPEAL FORM. Please see Mrs. Kuzmar in the Phys-Ed office for more information.

All athletes that plan to participate in an individual or team sport must read, fill out and have a parent/guardian sign this form.

Volunteer Driver Forms
If you wish to participate as a volunteer in any capacity throughout the school year, we will require a Criminal Record Check with Vulnerable Sector Screen to be provided to our main office. This can be completed online and will be kept on file for the school year. Those who wish to drive students to and from events will need to submit the "Authorization to Transport Students Participating in School Events" form.
Students who wish to drive other students are fill required to fill out the "Student Volunteer Drivers" Form A and all passengers must fill out the "Student Passengers" Form B
Please Note: Student passengers riding in a bus, taxi or adult volunteer drivers vehicle are not required to fill out a passenger form.
