Online CSIF

Helpful tips:

• Your username and password is the same as the log in for the online CPI 

• If you do not have an account set up or experience an issue, please email Bob Rabatin at rrabatin@mtaloy.edu or Emily Houseknecht at ehouseknecht@mtaloy.edu

• Once logged in, click on the 20xx APTA Clinical Site Profiles Surveys and complete the online CSIF

• Please be sure all fields are completed and the CSIF is signed off when you are finished. Thank you!
