Physical, Immunization, Dental and Vision Information

• All students entering grades Pre-K, Kindergarten, 6th, and 9th grade are required to provide new and updated physicals and immunizations by the first day of school. If the required documentation is not provided, your child will be excluded from school attendance beginning the first full day of school. 

• The due date for students new to the district after June 1 for physical examinations and immunization records is 30 calendar days after the first attendance day 

2024-2025 DENTAL REQUIREMENTS

All 6th grade students MUST have a dental examination dated after November 15, 2023 on file with the school by May 15, 2025.

A dental program will be offered at WES in March 2025 through Dental Safari. VISION REQUIREMENTS

All Kindergarten students and students new to Illinois schools must have a vision exam on file.
