First Day of School Year Requirements
6th grade: The due date for incoming 6th graders for physical examinations and immunization records and/or updates is the first full day of school (August 15, 2025).
Other Health Requirements
All New Students: The due date for students new to the district after June 1 for physical examinations and immunization records is 30 calendar days after the first attendance day.
6th grade: Dental examination completed by May 15th of current school year.

2025-2026 DENTAL REQUIREMENTS
All 6th grade students MUST have a dental examination dated after November 15, 2024 on file with the school by May 15, 2026.
A dental program will be offered at MMS in March 2026 through Dental Safari. There is an electronic consent form link below titled "Dental Safari Electronic Consent" that can be completed and submitted.
