How To Sign Up

NOTE Regarding 6th grade students : 6th Graders ARE allowed to sign up for team sports at Noble Middle School. 6th grade athletes will be allowed to practice with the 7th grade teams only, and will not be eligible to participate in games. This is in accordance with the SMMSAC league rules. Some sports allow for 6th graders to compete (Cross Country, Wrestling, Indoor Track). 6th graders still pay the activity fee sport participation. For more information regarding 6th grade participation in team sports, please contact Assistant Athletic Director Paul Slovenski.

Before Student-Athletes may practice, play or be issued equipment, the following must be completed and submitted to the Athletic Office:

1. Sports Physical Examination Form: Must provide evidence that the student-athlete has had a physical within the past two years and has been cleared by a physician to participate in the school’s athletic program(s). This form, or a copy of a physical from the physician's office may be emailed to tanya.ouellette@msad60.org or alexandro.fusco@msad60.org or faxed to 207-676-2829.
2. ATHLETE PARTICIPATION FORM
   • This form must be completed once per sport season. Please note that insurance information must be filled out in order to complete the Athlete Participation Form.
3. Activity Fee: The activity fee for each sport is due at the beginning of the sport season (no later than the first competition). There are three methods to pay the activity fee:
   1. My School Bucks (online payment system)
   2. Check made out to MSAD 60 Athletics
   3. Cash
