Transportation Information

*Top portion of this form must be complete for every student-athlete participating in the Spring Softball Season (multi-sport athletes should indicate all season).

*Bottom portion to be completed by anyone planning to transport any student-athletes other than their own to tournaments and or home from road games when a bus is provided and or to or from games when a bus is not.

*If transporting any student-athletes other than your own a copy of your drivers license and insurance policy showing the minimum coverage requirements listed on the form must be attached.

*Any student-athlete planning to be transported by someone other than their parent or guardian or the school provided bus must check yes on Question #2.

*Any student-athlete being approved to drive themselves when a bus is not provided must also submit a copy of their DL and insurance policy showing the minimum required coverages (student-athletes may only transport themselves to and from tournaments and no other student-athletes).
