Instructions: Print and complete the Registration Forms Checklist along with all required (and applicable) forms from the list of Medical Forms and list of School Forms. Once you have completed the pre-registration, printed and completed all forms, along with the checklist, and have gathered the necessary proofs of residency and original birth certificate, please visit the school at which you are registering your child to complete the registration process. The Main Office at both schools is open Monday-Friday from 8:30 AM-4:00 PM (please visit www.mpsdk8.org for the current school year calendar).
