MnCHOICES assessment determines whether an individual with a disability needs services and supports to live and work in their communities. The assessment helps decide the types of support that someone might be able to access, as well as how much support they need, and next steps to take to begin accessing services. This assessment can help determine if the individual qualifies for PCA services, or waivers like the CADI or DD.Â
