PARENTS’ GUIDE TO IMMUNIZATIONS REQUIRED FOR SCHOOL ENTRY and ADVANCEMENT TO 7TH GRADE
Students Admitted at TK/K-12 Need:
Diphtheria, Tetanus, and Pertussis (DTaP, DTP, Tdap, or Td) — 5 doses
(4 doses OK if one was given on or after 4th birthday.
3 doses OK if one was given on or after 7th birthday.)
For 7th-12th graders, at least 1 dose of pertussis-containing vaccine is required on or after 7th birthday.Polio (OPV or IPV) — 4 doses
(3 doses OK if one was given on or after 4th birthday)Hepatitis B — 3 doses
(*Not required for 7th grade entry)Measles, Mumps, and Rubella (MMR) — 2 doses
(Both given on or after 1st birthday)Varicella (Chickenpox) — 2 doses
These immunization requirements apply to new admissions and transfers for all grades, including transitional kindergarten.All Students Starting 7th Grade Need:
• Tetanus, Diphtheria, Pertussis (Tdap) —1 dose
(Whooping cough booster usually given at 11 years and up)
• Varicella (Chickenpox) — 2 doses
(Usually given at ages 12 months and 4-6 years)*In addition, the TK/K-12 immunization requirements apply to 7th graders who:
previously had a valid personal beliefs exemption filed before 2016 upon entry between TK/Kindergarten and 6th grade
are new admissions
Immunization Records:
California schools are required to check immunization records for all new student admissions at TK/Kindergarten through 12th grade and all students advancing to 7th grade before entry. Parents must show their child’s Immunization Record as proof of immunization. Records must include:
Student's legal name
Date of birth
Name of clinic or agency issuing the immunization record
**If immunization records provided do NOT include the above information, they will not be accepted.

How to Request a Medical Exemption (CAIR-ME)
Starting January 1, 2021, all new medical exemptions for school and child care entry must be issued through CAIR-ME. A doctor's note or documentation of a lab drawn titer is no longer acceptable. Parents can create an account in CAIR-ME and apply for an exemption. They will receive an exemption application number to give to their child’s physician. The physician can register for an account in CAIR-ME and log in to issue the ME. Once the ME is issued, the physician prints the form and provides a copy to the parents to give to their child’s school. These clinics offer free and/or low cost health exams and immunizations.
California passed a law on July 1, 2016 eliminating the religious and personal belief exemptions for immunizations. Unless your child has a valid medical exemption, he or she will not be able to enter kindergarten without having all recommended shots. Students with existing exemptions will be allowed to continue in school until his or her next vaccine check, which happens in kindergarten, seventh grade, and any time a student changes schools.
If you choose to not have your child vaccinated, he or she will not be allowed to attend public or private school in California and must enroll in homeschooling or independent study (SMCOE, 2023).
California aprobó una ley el 1 de julio de 2016 que elimina las exenciones por creencias religiosas y personales para las vacunas. A menos que su hijo tenga una exención médica válida, no podrá ingresar al jardín de infantes sin tener todas las vacunas recomendadas. They are licensed by the Food and Drug Administration (FDA) and go through rounds of review and clinical trials to ensure a high degree of safety. As with all medicines, there are risks of serious side effects, but those risks are small compared to the benefits of being vaccinated. Lastly, there is no link between vaccines and autism, and the idea that there are negative health effects when multiple vaccines are given at the same time has been disproved (SMCOE, 2023).
