Please follow the steps below if it has been determined that your case meets the State criteria for a referral to complete a substance use assessment.
These steps are also to be followed if you require an "MO Rec" (multiple offended recommendation.)
IMPORTANT: IF YOU ARE CURRENTLY IN IDRC CLASS NOW:
Your IDRC referral paperwork must be completed and returned before the end of class on the last day.
Failure to submit your paperwork will result in no credit being issued for your class attendance.
Step 1 – Locate a Provider for a Substance Use Assessment:
To remain compliant with IDRC requirements, you must complete a substance use assessment with an outpatient treatment agency or a licensed substance use clinician.

If you are currently in treatment or have completed an outpatient program within the last 12 months, please provide the name and contact information of the agency on the referral form in the next step.
Kindly ask your treatment provider to submit documentation to our office within 10 days at help@dui12.com.
Step 2 – Submit Referral Paperwork:
Once you have selected a provider, please complete the online Records Release and Referral Form by clicking the link below:
Be sure to include the provider’s name, contact information, and email or fax number on the form. If you do not provide this information, we cannot submit the referral!
Step 3 – Notify Our Office of Your Appointment:
Once your assessment appointment is scheduled, please email help@dui12.com with your appointment date and location so we can provide a favorable and accurate update to the court on your compliance status.
Send a new email with your name in the subject line, and let us know where and when you are going.
Please note:
Your assessment must be scheduled within 10 business days.
Your assessment must be completed within 30 calendar days to remain in compliance with your court-mandated IDRC requirements.

At Your Appointment PLEASE Ensure Provider Submits Documentation
Your treatment provider must send your assessment results and any treatment documentation directly to the IDRC office. This documentation may be submitted:

If your case is currently in non-compliant status due to missing your previously scheduled, court-mandated IDRC class, it will remain in non-compliance with the court of conviction and MVC until all IDRC program requirements are fully completed, including:
Completion of the assessment
Completion of any treatment that may be recommended
The IDRC cannot issue a report of compliance to MVC until these steps have been satisfied. If your driving privileges are currently suspended, they will remain suspended until your IDRC requirements are completed in full.
