Handbooks and Forms

In this section we will link the different handbooks and forms. Each student and parent are expected to know the rules of our team and understand them. Parents, please sign and return the acknowledgement of the team handbook form as well as the bus waiver form to Coach Malecki as soon as you can. Students you will be quizzed about your handbook throughout the season. Make sure you know it inside and out.

Form - Team Bus Pickup

Please read this bus waiver. Coach Malecki will not release a student into parent's custody without this waiver being read and signed by parents. If you wish to pick your kids up from a tournament or dual match, you must follow these policies.
