The registration and forms below are required for students to participate in Montini athletics. The registration is a Google Form and can be filled out online. The other four forms need to be signed and returned to the office.
Please click the link to register your student(s) for the sports season. One form for each student-athlete.
If you are interested in coaching, please contact Mr. Semcken.
Student-athletes must have a completed yearly physical to participate in sports.
Student-athletes and parents/ guardians must complete the following consent form along with written statement from a physician that is is safe for the student to return-to-play and return-to-learning.
