Registration/Forms
The registration and forms below are required for students to participate in Montini athletics. The registration is a Google Form and can be filled out online. The other four forms need to be signed and returned to the office.
Please click the link to register your student(s) for the sports season. One form for each student-athlete.
If you are interested in coaching this is the same registration form.
Student-athletes must have a completed yearly physical to participate in sports.
Student-athletes and parents/ guardians please read and sign the power of attorney form.
Student-athletes and parents/ guardians please read and sign the expectations form.
Student-athletes, parents/ guardians both must read and sign the concussion policy to be able to play sports.
Student-athletes and parents/ guardians must complete the following consent form along with written statement from a physician that is is safe for the student to return-to-play and return-to-learning.
