FLIP FLOPS:

Please make sure your child is wearing shoes that are strapped on to their feet. Some examples would be tennis shoes and sandals. Your child will not be allowed to play on the playground equipment if they wear flip-flops or shoes without a back to them. For more information, contact the school or Ms. Cindy.

ITEMS REQUIRED FOR PRESCHOOL:

1. Immunization Certificate (up to date)

**A copy of this must be on file at school before the child's first day!!

2. Copy of official Birth Certificate

3. Kentucky eye exam

4. Physical exam (You have 30 school days to complete this from the 1st day your child begins school.)

*Bring these items in when you enroll your child. You can also fax them to JHC at 270-487-9515.

SUPPLIES NEEDED FOR SCHOOL:

Regular sized backpack

Change of clothes including underclothes, socks, top, and shorts/pants

Cover and small pillow
