Physical exam -students must have a physical exam on the correct form completed at least one (1) year prior to enrollment. Anyone participating in sports at MS or HS needs a separate sports physical as well.

Immunization Certificate which includes the following shots:

• Meningococcal Vaccine-one dose of Meningococcal vaccine for 6th grade entry, 11 or 12 years or older is required.

• Tetanus-Diphtheria-Acellular-Pertussis (TDAP) - One (1) does regardless of interval since the last dose of tetanus-containing vaccine is required for students at 6th grade entry, 11 or 12 years older, with an option for TD for individuals who cannot receive pertussis- containing vaccines.

• Varicella- a second dose is required for children entering the 6th grade, 11-12 years of age or older if these have not already been vaccinated with two (2) doses of varicella.

• Two (2) Hepatitis A vaccines.
