This new project aims to increase access to community supports and clinical services for students and their families. The service will provide onsite access to OCHS navigator services and tele-behavioral and telemedicine appointments.
The school-based OCHS Navigator will work alongside school personnel to help students and families to access locally-based services and resources. The Navigator program will support students in connecting to the resources that they need to improve health, wellness, and academic engagement. Navigators will provide customized case management services, ensuring that students and families are connected to the supports that they need.

Jennifer is a dedicated Program Manager and Community Health Advocate with extensive experience in coordinating care for youth, families, and underserved populations. She has spent her career working across school-based, community, and clinical settings, with a strong focus on expanding access to care and addressing health-related social needs.
Jennifer currently oversees the Community Health Worker Department at Outer Cape Health Services, managing a range of programs including school-based health services, chronic care management, and outreach and engagement initiatives. She has a deep background in public health programming, practice management, systems navigation, and strategic partnership development, and has served on multiple countywide workgroups and advisory councils focused on youth mental health, behavioral health access, and rural health equity.
Her passion lies in creating sustainable, trauma-informed systems of care that center the voices of those most impacted. Jennifer’s approach is grounded in equity, collaboration, and practical problem-solving—ensuring services are not only accessible, but truly responsive to the needs of the community. Whether advocating for a student, supporting a caregiver, or designing a workflow, Jennifer is committed to building bridges between people and the care they deserve.
**Tele-medicine services are available to new or existing OCHS primary care patients. The school-based navigator may assist students with non-OCHS primary care providers in accessing services at their regular non-OCHS provider’s office.
Clinical services with medical and behavioral health clinicians will be charged to patient insurance and may be subject to cost-sharing (copays, deductibles, etc.). Students and/or guardians may contact the navigator for assistance accessing financial support resources.
