Lift Pass Application

To apply for Lift Pass, please follow the steps below:

• Click on this link to find your Character Coach's email address. 

• Click on the button to download the application form       ⏩⏩⏩⏩

• Click on the email address to open up your email.  

1. Follow the template below.

2. Attach the Lift Application Form to the email

3. Attach any medical certificate or doctor's letter if you have one.

4. Check that you are sending to the correct person before clicking on Send button
