Greetings from Rivervale Primary School!
1) Congratulations on the successful enrolment of your child into Rivervale Primary School for the Year 2025.
2) Please refer below to the list of documents for completion and submission.
3) Please note that Items 1 - 8 are compulsory while items 9 -12 are optional.
Student Dismissal Arrangement (compulsory)
Parent has to complete this P1 Student Dismissal Form latest by 19 Dec 2025.
2. Student Details Form (compulsory)
MOE has an online “Student Details Form (SDF) portal” for capturing the particulars of children who will be admitted to Primary One in year 2026.
This portal will be accessible to parents from end Nov 2025.
Only one parent (father or mother) is required to submit the student details to MOE by 31 Dec 2025.
3. Primary 1 2026 Name Tag Order (compulsory)
Open date for Online Purchase of P1 Name Tags is from 10 Oct 2025 to 20 Oct 2025. These ordered Name Tag will be ready by 05 Dec 2025. These name tags will be distributed to P1 Parents when they collect the textbooks in school OR delivered to P1 Parents when they choose home delivery for textbooks.
Please click Name Tag Order to purchase name tags for your P1 child.
Please ensure that your child’s name on the name tag must be a registered name as reflected in his/her Birth Certificate.

This portal will usually be accessible to parents at end December.
9. Non-Mother Tongue Language Form
To be submitted only by students who are:
- Not taking their Mother Tongue Language
- Not taking any of the official 3 Mother Tongue Languages (Chinese, Malay or Tamil)

10. Application for Financial Assistance Scheme (FAS)
The FAS Scheme is for Singapore Citizen students with Gross Household Income of (TBA) and below OR Per Capita Income of TBA and below.
e-FAS form is available on Parents Gateway (under Services tab – Financial Assistance) via Singpass login or you can access the e-FAS from this: TBA
11. Application for School-based Student Care Centre (Pro-Teach Education Centre)
The school-based student-care centre (SCC) provider is Pro-Teach Education Group.
If you would like to enrol your child in the SCC, please click here to fill in the form.
All applications to be submitted by 24 October 2025
