1) Congratulations on the successful enrolment of your child into Rivervale Primary School for the Year 2024.
2) Please refer below to the list of documents for completion and submission.
3) Please note that Items 1 - 6 are compulsory while items 7-10 are optional.
Student Data Form (compulsory)
MOE has an online “Student Details Form (SDF) portal” for capturing the particulars of children who will be admitted to Primary One in year 2024.
This portal will be accessible to parents from 28 November 2023.
Please complete the form by 29 December 2023.
2. Application Form for Interbank GIRO (compulsory)

GIRO will be used for your child/ward’s payment of school fees, enrichment programmes and school charges.

4. School Uniform (compulsory)
Please refer to the attachment sent out on Parents Gateway (on 17 Oct 2023) for school uniform details.

Please click here for online purchase of your child/children's textbook and name tag order.
The website will be launched on 6 November 2023.
Please ensure that your child’s name on the name tag must be a registered name as reflected in his/her Birth Certificate.
Name tags will be distributed by your child’s Form Teachers when school reopens.
P1 2024 Booklist.pdf6. School Health Services and School Dental Checks (compulsory)

Parents MUST give your consent before your child can receive the services.
This portal will usually be accessible to parents at end December.
7. Non-Mother Tongue Language Form
To be submitted only by students who are:
- Not taking their Mother Tongue Language
- Not taking any of the official 3 Mother Tongue Languages (Chinese, Malay or Tamil)

8. Application for Financial Assistance Scheme (FAS)
The FAS Scheme is for Singapore Citizen students with Gross Household Income of $3000 and below OR Per Capita Income of $750 and below. Please refer to the attached for the pamphlets for more information.
e-FAS form is available on Parents Gateway (under Services tab – Financial Assistance) via Singpass login or you can access the e-FAS from this: https://go.gov.sg/moe-efas.

The school-based student-care centre (SCC) provider is Pro-Teach Education Group.
