To make an appointment with Mr Daniel Lum:

1. Go to https://go.gov.sg/adss-ecg

2. Enter your phone number. A one time password would be send to you.

3. Select your preferred date and time.

4. Complete and submit the Appointment Booking Form.

5. Check your email for the confirmed appointment slot. You will receive an email from Mr Daniel Lum within 3 working days.

6. Attend the ECG Consultation Session.
