Client Responsibilities
Online documentation must be completed prior to your first appointment. If you have sufficient reasons for not being able to compete documents online, you may completed them on paper, in person; however we request that you arrive 30 minutes early.
You are responsible for knowing your deductible, co-pay, and mental health benefits prior to your first session.
If you have questions, you may call the customer service or member services line typically found on the back of your insurance card. If you schedule a phone consultation or an appointment please bring this information.
Out-of-Network
If you have out-of-network benefits, you may request a superbill from your therapist to submit to your insurance provider for reimbursement.
