Middle Path closed February 1st, 2026.

At the time of closure, all current and former clients were notified through their client portal and by email that Middle Path had retained Cariend, a medical records custodian, to manage the practice’s records for the next 7 years.

The information below applies to all requests for records, including requests from current or former clients and requests made by third parties such as disability carriers, insurance companies, attorneys, healthcare providers, government agencies, and other authorized requestors.

Middle Path no longer maintains an active office, telephone line, or email account for records requests. Please do not contact former staff, clinicians, property owners, or any previous business address, telephone number, or email address regarding records. All records requests and records-related inquiries must be directed to Cariend using the information below.

Cariend is not a medical provider and does not handle billing, medications, or referrals for the closed practice.

Requestors should be prepared to complete the Records Release Authorization Form and provide a copy of government-issued photo ID, as required by HIPAA.
