To request your Morris Hills High School transcript, please fill out and sign the attached form and return to Mrs. Mesa (hmesa@mhrd.org) via email.
You can fill out the form digitally, or print the form, take a picture and send it as a PDF. (*If you do not have access to the necessary technology, you can also include the requested information in the body of a response email.*)
You do not need to fill out the second page if you are only requesting that your transcript be sent to one place.
We will have your request processed within 3-5 business days.
