NC Health Assessment - completed within 30 days of enrollment
Will accept sports physicals OR well visit summary from doctor's office within last year.
If you have been in a NC public school at any point previously, you DO NOT need to complete the NC Health Assessment form
Immunizations - it is the parent's responsibility to provide an immunization record. This record must be from the health department or doctor's office. Please note that NC may have different requirements than other states. This completed record is due within 30 days of enrollment.
SENIOR IMMUNIZATION REQUIREMENT
Seniors are required to have a Meningitis vaccine to start the school year. Starting in August of 2020, a 2nd dose of the Meningococcal (MCV) vaccine is required by North Carolina before entering 12th grade. There are two Meningitis vaccines available, Meningitis B is recommended while Meningococcal conjugate vaccine (MCV) is required. Please send proof of vaccination by the first day of the school year.
In the past, some urgent care and pharmacies did not update the NC Immunization Registry so please ask for paper documents if you choose these locations. Please do not wait until August to get this immunization as you may not be able to find it due to the number of people that wait until the last minute.

Iredell County Health Department - take immunization records with you
Statesville location - They have appointments available but can also accept walk-ins.

This is a NC State Law to comply with vaccination requirements. If your student does not have documentation of this required vaccination, your student can be excluded from school with unexcused absences until proof of immunization is received by the school.

Medication Forms:
Medication Authorization Form - This must be filled out for student's to received medication at school. Medication must be checked into the school by a parent or guardian.
Over The Counter Form - Middle and High School students can self carry and administer over the counter medications. Please turn this form into the nurse.
COVID INFO and Forms
MGSD recommends following the CDC guidelines for testing positive for covid-19. Please email MHSattendance@mgsd.k12.nc.us for documentation of the absence. These absences must be reported within 3 days of returning to school.
A doctor's note is not required but is recommended if possible. A parent should be the one to report the covid absence, not the student.
Please have the student email their teachers and keep up with their work in Canvas when they are feeling better.
Health Conditions:
If your child has a health condition that needs special assistance and/or an emergency action plan at school, please contact your school nurse. Examples of health conditions that the nurse needs to talk to parents about include diabetes, asthma, severe allergy, seizures, etc. The nurse will work with school staff to ensure that your child’s special health care needs are met.
