INJURIES

• If you become injured at practice you must notify a coach immediately
• It is highly recommended that you see a sports medicine doctor before going to see your regular doctor

• Lake Health injury clinic is now at the Lake Health building by Mentor HS. This is now by appointment only
• If a doctor has restricted you from participation you must have a note from that doctor clearing you to play.
• Parents cannot write a note clearing their child to participate

Concussions

• Coaches will follow concussion protocol when dealing with head injuries and concussions.
• If a doctor diagnosis your child with a concussion they will not be able to participate until the doctor signs the concussion release form
