Emerson Athletics 

• All student athletes will need a physical within the last 12 months  

• All Students must fill out the RANK ONE form before being able to practice.

All Athletes 6-8th Grade: 

• Must complete Rank One Form 

• Have an up to date physical and insurance on record in our Nurses office 

• Athletes may also be asked to complete a concussion Pre-test 

(to help us better keep your kids safewe will know more soon)

All 5th Grade Athletes:

• Must complete Rank One Form 

•  Have your doctor fill out this form SPORTS PHYSICAL FORM. 

• Return to the Conners office with a copy of your insurance card or proof of insurance. 

• Copy of permission form.
