Before You come to the Physical, please have the following completed:
Complete the Online Athletic Permission and Participation Form under the Online Form Submissions tab.
Print the Physical Forms from the Athletic Forms tab and have a parent/guardian complete and sign the History Form (pages 1-2).
Bring the completed History Form (pages 1-2) forms to your scheduled physical.
IMPORTANT: Physicals are valid for 365 days from the date of the physical.
