If you move please notify the Registrar immediately and provide proof of residency. Examples of acceptable documents can be found in the MCPS Quick Guide to Enrollment 560-24B
Please contact your child's counselor and complete this Withdrawal Statement and submit it to the Registrar at least 3 days before the student's last day.
Please note that these forms are typically updated at the beginning of each school year.
Senior Transcript Request Form
If you graduated or withdrew LESS than five years ago contact Mrs. Fedders (Registrar at PHS) to obtain your transcript (240-740-2417). If you graduated or withdrew MORE than five years ago fill out Student Record Request Form 226-8. Your transcript/diploma request must include:
1. Complete PART A only, print out form and sign where indicated
2. Please visit the SchoolCash Online system to make your payment electronically. Other acceptable payment forms: cash, check or money order payable to "MCPS". The fee per transcript is $6.00 and $16.00 per diploma, payable to "MCPS"
3. Transcript requests: Visit the Central Records web page for more information on how to make your transcript and/or diploma request.
4. Diploma Requests: Include mailing address for diploma (no envelope necessary) Please allow up to 8 weeks for replacement diploma processing.
