Medication 

Medication will NOT be accepted the day of the trip. 

Please bring any necessary forms and medications to the health room by March 14th, 2025. 

Medication

All medication and complete forms must be submitted to the health room by March 14, 2025.  

Medication will NOT be accepted after that date.   Medication that is already in the health room for this school year will automatically be sent along with the program, and no additional information is required.  

Please read the following if your child requires medication that is NOT already in the health room:

If prescribed medication is needed, MCPS form # 525-13 must be filled out by your child’s physician.   If your child requires non-prescription (over-the-counter) medication during their stay, you must complete Part 1 and Part 2.   All medications (prescription or over-the-counter) must be brought in SEALED, UNOPENED containers by a parent/guardian along with the MCPS form # 525-13 completed.  If you have any questions, please contact the school nurse at 240-740-4152. 

 Medicamentos Todos los medicamentos y formularios completos deben enviarse a la enfermería de la escuela antes del 14 de marzo del 2025. No se podrá aceptar medicina después de esa fecha. Los medicamentos que ya están en la enfermería de la escuela para este ciclo escolar se enviarán automáticamente junto con el programa y no se requiere información adicional. 

Lean lo siguiente si su hijo/a requiere medicamentos que NO están ya en la enfermería de la escuela: 

Si se necesita un medicamento recetado, el médico de su hijo debe completar el formulario # 525-13 de MCPS. Si su hijo necesita medicamentos sin receta (de venta libre) durante su estadía, debe completar la Parte 1 y la Parte 2. Todos los medicamentos (recetados o de venta libre) deben ser traídos en recipientes SELLADOS y CERRADOS por un padre/tutor junto con el formulario MCPS # 525-13 completado.
